New and renewed license certificates are emailed to the regulatory contact person on record at the time the license is issued, or renewal is released complete. This is your official license certificate. It is advisable to check the junk/spam email boxes of the regulatory contact. If there is a change in the regulatory (facility or DRP) contact person, report the change via the Board’s change of license information request format available on this website – License Info Changes page. Per Board regulation, change of license information for a current license must be submitted in writing within 60 days of the effective date of the change.

A duplicate license certificate may be requested in writing submitted to the Board’s administrative office along with payment of the $10 duplicate license certificate fee (per LAC 46:XXXIV.801.A.4) by check or money order.
